Boone County’s Age & Disability Waiver Program is for income-eligible individuals who need long-term care, but desire to remain at home rather than reside in a nursing home or medical facility. Long-term in-home care may be an option for residents of Boone County as a nursing home alternative, if determined to be eligible for this program.

Case Management Services
- Case managers can help match seniors to available services
- Evaluation of needs
- Assist with advocacy
- Oversee and monitor provided services
- Help seniors maintain independence
In-Home Services
- Nursing staff, personal care attendant, and case manager provided
- Services can include errand running, personal care, nursing, housekeeping, recreational activities
Homemaker Services
- Alternative to Nursing Home care
- Maintain independence
- Nutrition and meal preparation, shopping/errands, light housework, emotional support, and money management.
Eligibility:
The Age & Disability Waiver Program is available to residents of Boone County, WV age 18 years or older who meet the program eligibility requirements. Individuals must be medically eligible for a level of care provided by a nursing home and the applicant, along with their physician, must complete an evaluation form. Individuals must also be determined to be financially eligible, Medicaid-eligible, by the West Virginia Department of Health and Human Services.
