Boone County’s Personal Care Program assists those with physical and/or mental impairments with a variety of services in-home.  We realize that the needs of individuals will vary greatly, so different program options are available.  The goal of these services is to provide quality, individualized medical and personal care support in the comfort of the home environment.

Personal Care Services:

  • Are ordered by a physician according to a nursing plan of care, out-patient instead of in a facility
  • Can include feeding/nutrition, bathing, personal hygiene, toileting, dressing, transferring in/out of bed
  • Are provided by professionally trained personal care staff, overseen by an R.N.

Homemaker Services:

  • Are direct and practical assistance with household tasks and related activities
  • Assist individuals who have lost the ability to perform instrumental activities of daily living that allow them to live in a clean, safe, and healthy home environment
  • Are available when the individual is unable to meet daily needs and there is no informal caregiver who could meet those needs
  • Activities can include dusting, vacuuming, mopping, doing dishes, laundry, making beds, disposing of trash, grocery shopping, preparing meals, running errands and other household services as needed
  • Must be 60 years or older to qualify

Personal Care Plus Homemaker Services:

  • Are ordered by a physician according to a nursing plan of care, out-patient instead of in a facility
  • Include Personal Care services listed above, plus additional services such as dusting, vacuuming, mopping, doing dishes, laundry, making beds, disposing of trash, grocery shopping, preparing meals, running errands and other household services as needed
  • Are provided by professionally trained personal care staff, overseen by an R.N.

Eligibility:

  • Individuals must be medically and financially eligible for our Personal Care or Homemaker Services
  • Individuals who are eligible for Medicaid may be eligible
  • Must complete a medical assessment performed by a physician
